What are dermal fillers?

Dermal filler injections offer a non-surgical cosmetic solution to plump up wrinkles, smooth lines, and restore facial volume. Administered just beneath the skin by a healthcare provider, these fillers are chosen by individuals seeking to enhance their facial features or achieve a more youthful look. The procedure, typically lasting under thirty minutes, involves minimal recovery time, and results are immediately noticeable, lasting from months to years depending on the filler type and application area.

Various types of dermal fillers are available, and your healthcare provider will guide you through the options. However, like any cosmetic procedure, there are potential risks such as infection, bleeding, and bruising. It's important to note that elective cosmetic procedures like filler injections are typically not covered by health insurance plans.

Why are dermal fillers used?

As you age, collagen depletion can lead to skin laxity, loss of volume, and decreased elasticity. Dermal fillers can be opted for to:

  • Restore volume to sagging skin.
  • Enhance facial symmetry.
  • Add fullness to lips and cheeks.
  • Smooth out wrinkles and creases.
  • Some individuals may consider dermal fillers to counteract facial thinness resulting from medications used to treat conditions like HIV and AIDS.

What are the types of dermal fillers?

There is a wide range of dermal fillers available, including "off-the-shelf" fillers made from synthetic materials or naturally occurring substances found in the body. Another option is autologous fat grafting, where healthcare providers extract fat from one area of your body and inject it into your face.

  • Hyaluronic acid (HA) is a natural acid found in your skin that provides volume and hydration. As you age, your body produces less HA. HA injections, such as Restylane®, typically last from six months to a year.
  • Calcium hydroxylapatite (CaHA) fillers, like Radiesse®, are composed of a substance found in bones. These fillers are effective for deeper wrinkles and can last about a year.
  • Poly-L-lactic acid (PLLA) helps stimulate collagen production in your body. PLLA fillers, like Sculptra®, are used to smooth deep wrinkles and can last two years or more.
  • Polymethylmethacrylate (PMMA) fillers, such as Bellafill®, contain collagen and small beads that remain under the skin to provide volume and firmness.

Your healthcare provider will assess your needs and discuss the most suitable dermal filler option for you, setting realistic expectations and outcomes for the injections.

What happens before a dermal filler procedure?

Before receiving dermal fillers, you'll undergo a consultation with your healthcare provider, which may involve a dermatologist specializing in skin care. They will evaluate your face, discuss your goals, and identify areas you want to enhance.

Your provider may mark your face and take photos to plan the injections. They will recommend suitable fillers, explain potential side effects, and discuss recovery.

During the consultation, your medical history will be reviewed. Inform your provider about any medications you're taking, as some may increase risks. Mention if you have:

  • Allergies
  • History of bruising or bleeding post-procedures
  • Neurological issues
  • Skin conditions

What happens during a dermal filler procedure?

Dermal filler procedures are commonly performed in healthcare provider offices or medical spas, also known as medspas or medispas. These facilities offer cosmetic treatments in a relaxing spa-like setting.

Before the procedure, your healthcare provider will cleanse your skin and may apply a numbing lotion or cream for comfort. This anesthetic helps alleviate any discomfort during the treatment.

Using a fine needle, your provider injects small amounts of filler beneath your skin. While the needle may cause slight pinching or stinging sensations, most individuals tolerate the injections well. Multiple areas may be targeted during the session, which typically lasts from a few minutes to an hour.

What happens after a dermal filler procedure?

Following dermal filler injections, your healthcare provider will cleanse your skin and may provide an ice pack to alleviate any pain or swelling.

It's common to experience bruising, swelling, or mild discomfort post-injections, which typically subside within a few days.

While some individuals notice immediate results, others may see changes gradually. The duration of results varies based on factors such as the type of filler used and individual response to treatment.

What’s the process for a facial fat grafting procedure?

Facial fat grafting involves a distinct process. Initially, your healthcare provider performs liposuction to extract fat from your body. After purifying the fat, it's injected into your face. Typically done as an outpatient procedure, you can return home the same day.

Given the additional step of liposuction, this procedure is more complex compared to receiving off-the-shelf dermal fillers. After liposuction and facial fat grafting, you may require up to two weeks of rest. Consult your healthcare provider regarding recovery duration and suitability for this procedure.

What are the risks or complications of dermal fillers?

Serious complications from dermal filler injections are uncommon, and many side effects are temporary. However, like any cosmetic procedure, there are risks involved:

  • Asymmetry in appearance.
  • Potential for bleeding, bruising, redness, pain, and swelling.
  • Risk of skin damage leading to scarring.
  • Possibility of infection, which in severe cases can cause skin necrosis (death of infected tissue).
  • Formation of lumps or bumps under the skin.
  • Temporary numbness.
  • Occurrence of pimples resembling acne.
  • Onset of rash and itching.

Rarely, individuals may experience vision issues post-injection. If you notice vision problems along with weakness, discomfort, or one-sided body pain, seek immediate medical attention.
